- [36-32-5] Ceiling/ceiling tile shows damage or is in disrepair. Observed hole on ceiling tile above WIC entrance.
- [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed interior of ninja blender soiled at prep table at kitchen area. Unit was disposed off.
