  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink. Operator grabbed some napkins from across the MFDV.
  • [22-16-4] Reach-in cooler interior/shelves have accumulation of soil residues. Observed interior of fridge and freezer soiled.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed threaded hose Bibb on exterior of food truck missing a vacuum breaker.
  • [08B-38-4] Food stored on floor. Observed multiple plastic bottles of water on floor next to entry door.
  • [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee beverage on prep table next to cold bar. Operator removed beverage.
  • [29-24-4] Clean-out cap missing from plumbing line. Observed clean out cap missing on plumbing line of MFDV.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed milk, raw beef (45F - Cold Holding) in fridge. Operator stated she was in and out of fridge a lot this morning. Operator removed lids to help chill foods.
  • [35B-05-4] Outer openings not protected during operation and vermin and/or environmental cross contamination present. Observed outer opening of food truck unable to fully seal when closed and flying insects present.
  • [50-17-3] Operating with an expired Division of Hotels and Restaurants license. Observed MFDV operating and serving guests with an expired license. Operator stated she couldnt get into the portal. Inspector supplied info to use the automated system. Operator took down phone # to call
