[14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Observed buildup of ice in walk in freezer.
[36-64-5] Objectionable odors in bathroom or other areas of the establishment. Observed odors in both men and women bathrooms.
[50-17-3] Operating with an expired Division of Hotels and Restaurants license. Observed license expired 12/01/2025. Establishment is open, operating and serving the public with an expired license. Operator states they are locked out of account online. I provided the Help Line phone number for further assistance to alleviate problem.
[02C-02-5] Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed lasagna prepared two days ago per Operator stored in reach in cooler on cook line with no date mark.

[06-01-5] Time/temperature control for safety food thawed in an improper manner. Observed frozen fish thawing in buckets in kitchen, educated the operator of proper thawing techniques, suggested either under running cool water or under refrigeration. Operator moved fish to walk in cooler.

[12A-07-5] Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee return to line from walk in cooler without first washing their hands. Discussed proper hand washing procedures with the staff.
