- [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Two hose bibbs above triple sink missing vacuum breakers, hose Bibb outside of back door missing vacuum breaker. Hose Bibb with hose attached by walk in cooler missing vacuum breaker.

- [08A-20-5] Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In make line reach in cooler raw chicken stored over raw lamb. Person in charge began rearranging.
