  • [16-13-5] Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed dishwasher rinsing soiled items in sanitizer step of three compartment sink.
  • [36-17-5] Floor in disrepair. Observed for walk in cooler metal floors that area lifting and broken causing a tripping hazard.
  • [36-73-4] Floor soiled/has accumulation of debris. Observed for floors in walk in cooler.
  • [08B-38-4] Food stored on floor. Observed at dry storage area boxes of bananas stored on floor. Operator removed.
  • [10-07-4] In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed throughout, utensils in standing water of 72F. Operator removed.
  • [22-16-4] Reach-in cooler interior/shelves have accumulation of soil residues. Observed at reach in freezer where açaí is stored. Operator began to clean during inspection.
  • [25-06-4] Single-service articles not stored inverted or protected from contamination. Observed for to go boxes along prep counter. Operator inverted.
  • [36-27-5] Wall soiled with accumulated grease, food debris, and/or dust. Observed at walls above hand sink at cook line.
  • [42-01-4] Wet mop not stored in a manner to allow the mop to dry. Observed stored in mop bucket at dry storage area.
  • [01B-01-4] Dented/rusted cans present. See stop sale. Observed 1 dented can of corn. Operator segregated to discard.
  • [22-41-4] Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed Dishwasher (Chlorine 0ppm). Operator replaced empty container and primed machine new Dishwasher (Chlorine 50ppm)
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed at kitchen prep counter garlic in oil (63F - Cold Holding). As per operator less than 2 hours.
  • [02D-12-1] Juice packaged in the establishment that has not been specifically processed to prevent, reduce, or eliminate the presence of pathogens not bearing a warning label. Observed at lobby beverage coolers for prepared onsite assorted fruit and wellness juice not bearing the warning label. Operator began to print and place warning labels onto items.
  • [53A-05-6] No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/DBPR/hotels-restaurants/food-lodging/food-manager/ Observed 7 employees present engaging in food preparation with no certified manager present.
  • [02D-02-4] Packaged food not labeled as specified by law. Observed container of whit substance not label at prep counter shelf. As per chef was salt. Operator placed label.
