- [09-01-4] Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Server used bare hands to place washed and cut salad into a bowl. Manager stopped the employee and had them discard the salad, wash hands, and put on gloves before continuing to prepare salad.

- [13-06-4] Employee with artificial nails/nail polish working with exposed food without wearing intact gloves. Employee with painted and/or artificial nails preparing salad without wearing gloves. Manager had them employee put on gloves before continuing to handle food.

- [14-01-5] Bowl or other container with no handle used to dispense food. Single-use cup without handle stored down inside panko on shelf at kitchen entrance. Manager discarded the cup during this inspection.
- [08A-20-5] Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Pan containing raw chicken stored on shelf directly above portions of raw beef steak inside reach-in cooler at cookline. Manager moved the chicken to the bottom shelf during this inspection.
