[14-05-4] Cardboard used to line food-contact shelves. Observed soiled cardboard in dry storage area lining shelves with food on it.
[35A-02-7] Live, small flying insects found. Observed approximately 20 small flies in dry storage over two bags of onions and observed approximately 10 flies on the prep table and dish area.
[08B-38-4] Food stored on floor. Observed a pan of fish and a pan of steak on the floor in the walk in cooler and three containers of mix cheddar cheese on the floor in the walk in freezer, operator moved to shelve.
[45-05-4] Portable fire extinguisher missing from its designated location. For reporting purposes only. Observed fire extinguisher on the floor underneath hand wash sink by ice machine in kitchen and not in designated place.
[29-08-4] Plumbing system in disrepair. Observed one hand wash sink in the dish area draining into a five gallon bucket, and a second hand wash sink draining into a five gallon bucket in the kitchen.
[12A-09-4] Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed cook touched his apron while cooking and not change his gloves, operator spoke to the cook and educated on proper hand washing and changing gloves.
[02C-02-5] Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed pans of taco meat and refried beans not date marked in the walk in cooler, employee stated pulled from freezer-employee date marked.
