  • [16-23-4] Accumulation of lime scale on the inside of the dishmachine. Dish machine in dish area.
  • [14-01-5] Bowl or other container with no handle used to dispense food. Cup stored in cheese in reach-in cooler next to pizza oven. Manager removed cup during inspection.
  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee phone on prep table in front of pass through window. Manager removed phone during inspection.
  • [24-08-4] Equipment and utensils not properly air-dried - wet nesting. Cups stored in wait station.
  • [36-73-4] Floor soiled/has accumulation of debris. Floor underneath reach-in cooler next to pizza oven soiled with old food.
  • [10-17-4] In-use knife/knives stored in cracks between pieces of equipment. Knives stored in between reach-in cooler doors on cook line. Manager removed knives during inspection.

  • [25-05-4] Single-service articles improperly stored. Napkins stored on floor in storage area. Manager placed napkins on table.
  • [29-49-6] Standing water in bottom of reach-in-cooler. Reach-in cooler next to cook line fryers.
  • [29-20-5] Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Cook line hand washing sink slow draining.
  • [08B-12-5] Stored food not covered. Cookies stored in walk-in freezer.
  • [22-57-6] Heat strip failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface and/or high temperature holding thermometer did not reach 160F. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Dish machine in dish area, 155F.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Hose side of splitter in mop sink.
  • [11-27-4] Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. No written procedure available. Inspector provided operator during inspection.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Ice bin for wait station ice machine stained.
  • [31A-11-4] Handwash sink used for purposes other than handwashing. Money stored in bar area hand washing sink. Employee removed money from hand washing sink.
  • [31B-03-4] No soap provided at handwash sink. No soap and towels at bar area hand washing sink.
  • [53B-14-5] Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Two employee training certificates expired in early 2026.
