- [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine soiled with black mold-like substance.
- [12A-03-4] Employee washed hands in a sink other than an approved handwash sink. Observed employee wash hands in 3 compartment sink instead of handwash sink.
- [35A-03-4] Dead roaches/insects on premises. In back right corner of kitchen across from reach in freezer, 1 dead roach and 1 dead frog on floor. Employee cleaned up dead vermin and discarded.

In 3 crock pot unit on back prep table, chili (109F - Hot Holding); sour kraut (111F - Hot Holding). Employee stated chili and sour kraut were put into warmer about 11 am. Operator turned crock pots from warm to hot to bring temperature up.
