  • [06-09-1] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed in flip top at cook line , reduced oxygen packaged tilapia and swai fish not removed from packaging. Per operator item was taken out less than one hour ago. Operator removed items from packaging.
  • [14-09-4] Cutting board has cut marks and is no longer cleanable. Flip top cutting board at cook line.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Lower Interior of flip top at cook line.
  • [14-31-5] Nonfood-grade bags used in direct contact with food. Observed in fresh food reach in cooler in kitchen, raw chicken stored in printed to go bags. Operator removed from bags and placed in food grade pans .
  • [50-17-3] Operating with an expired Division of Hotels and Restaurants license. Observed establishment license expired since December 1st, 2025. Establishment is open and serving food to the public.
  • [05-08-4] No probe thermometer provided to measure temperature of food products.
  • [31B-03-4] No soap provided at handwash sink. Hand wash sink at cook line. Operator provided during the inspection.
