[16-37-1] No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Chef went to commissary kitchen and picked up both chlorine and quaternary test strips.
[36-34-5] Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance.
[51-11-4] Carbon dioxide/helium tanks not adequately secured.
[08B-36-4] Food stored in a location that is exposed to splash/dust. Observed a pack of crackers sealed in Plastic with water splatter from condensation leak from ceiling. Employee removed food items off of food contact surface and from affected area and placed on shelf.
[14-11-5] Equipment in poor repair. Ice machine in kitchen and at bar are not working. Technician is on site working on kitchen ice machine.
[01B-13-4] Stop Sale issued due to food not being in a wholesome, sound condition. Observed a Pack of crackers with condensation water splatter.
[28-02-4] Condensation or other drainage not disposed of according to law. Observed condensation from ceiling, light shield and AC vents dripping onto floor in buckets, and on food contact surface table. Observed water splatter on a pack of crackers sealed in plastic. Do not use affected food area until repairs are made..
