- [06-09-1] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Raw snapper thawed in sealed bags. Says it must remain frozen until you take it out of bag. You must cut open the bag while fish is still frozen.
- [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Glasses above food prep table. Person in charge moved glasses.
- [36-01-4] Floor not cleaned when the least amount of food is exposed. Grease on floor behind stove/fryers.

- [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In front reach in cooler; Butter 53F. Butter was sitting above fill line in cooler. Per person in charge it was just placed in cooler. Took butter and placed inside reach in cooler.
- [03B-01-6] Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Whole roasted chicken 113F hot holding. Placed chicken on time/temperature control for safety.
- [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Missing on hose at mop sink.
