  • [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee energy drink stored on prep table. Employee removed and stored properly.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter added to mop sink without back flow device on non chemical side of splitter. Operator placed ordered for back flow during inspection.
  • [22-49-4] Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chemical dish machine not sanitizing properly at 0ppm. Chemical bucket was empty. Manager changed out with new bucket of sanitizer. Second test at 100ppm.
  • [13-04-4] Employee with no beard guard/restraint while engaging in food preparation. Observed employee engaging in food preparation without beard guard.
