- [32-12-6] Covered waste receptacle not provided in womens bathroom. Observed for unisex bathroom.
- [05-09-4] No conspicuously located ambient air temperature thermometer in holding unit. Observed no ambient thermometer in reach cooler in the kitchen area.
- [16-13-5] Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized.
- [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed door gaskets of reach in coolers and freezers.
