  • [05-09-4] No conspicuously located ambient air temperature thermometer in holding unit. Observed no ambient thermometer in reach cooler in the kitchen area.
  • [16-13-5] Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed door gaskets of reach in coolers and freezers.
  • [14-74-7] Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed doors of reach cooler not closing properly, therefore not keeping food cold and ambient thermometer reading 50F and above. Observed multiple TCS food items out of temperature.   • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed build up of black substance on shelving unit housing clean pots above 3CS.
  • [33-16-4] Open dumpster lid.
  • [22-31-4] Non-pitting surface rust on food-contact equipment. Observed rust build up on shelves housing clean pots, pans and ladle above 3CS and counter shelf housing single use items in the kitchen area.
  • [33-11-4] Missing drain plug at dumpster.
  • [32-04-4] Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Observed for unisex bathroom.
  • [36-17-5] Floor tiles missing and/or in disrepair. Observed in the kitchen and prep areas.
  • [36-37-5] Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Observed ceiling tiles throughout kitchen and preparation area not smooth and easily cleaned.
