Disposition: Call Back - Complied
[23-03-4] Shelves holding racks of bread have been cleaned. Toaster had been cleaned. Lemonade station has been cleaned. The sticker on the bottom of the cabinet is still in the same location.
[36-34-5] Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed dusted vent over dry storage area.
[36-73-4] Floor tiles and grout are still in same condition. Floor underneath soda Bibb machine, preparation table and storage racks have been cleaned.
[14-33-4] Rack next to dish machine is still in the same condition. No rust observed on preparation table.
[22-02-4] Interior still soiled. Showed operator.
[38-01-4] Light shield damaged/in disrepair. Observed flickering light over preparation area next to dry storage area.
[16-46-4] Operator took label off one container with label left on. All containers from yesterday with labels were corrected yesterday.

Disposition: Administrative complaint recommended
[36-73-4] Floor soiled/has accumulation of debris. Floor area(s) covered with standing water. In prep area, inside walk in cooler , under prep table Crack in kitchen floor tiles observed in various locations throughout the kitchen. Floor soiled underneath dry storage racks next to the soda Bibb machine and underneath preparation table by employee bathroom. Floor observed to be not easily cleanable due to missing grout in various locations where water has been pocketed.
[41-18-4] Warewashing sanitizing solution exceeding the maximum concentration allowed. Three compartment sink in front area had sanitizer reading at 200ppm. Operator added water.
[28-13-5] Raw sewage on ground of establishment. Observed standing water on floor in back prep area ,under prep tables , under dry storage racks, and in walk in cooler. Operator was able to verify sink drain was not properly aligned with floor drain causing the gray water to flow on ground. Operator aligned drain and water is draining properly into floor drain. Operator cleaned water up , placed fans and sanitized areas.
[08B-17-4] Unwashed fruits/vegetables stored with ready-to-eat food. Observed unwashed mushrooms, romaine lettuce, and limes over ready to eat sliced tomatoes and sauces. Educated operator to move around storage.
[14-33-4] Dish drying shelves next to dish machine with rust that has pitted the surface. Rusted preparation table observed next to dry storage shelves. Operator started cleaning preparation table.
[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Interior of ice machine had build up of mold like substance.
[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed sticker on bottom of cabinet in customer self serve area underneath tea station with mold like substance. Lemonade station had build up of mold like substance behind where you push to serve in customer self serve area. Shelves holding racks of bread at preparation table near employee bathroom had build up of debris. Toaster had build up of food and debris
[24-07-4] Cleaned and sanitized equipment or utensils not properly stored. Manager states items are currently not in use. Pans, tea containers, single use straws and napkins. Informed the operator to move items out of employee bathroom.
[31A-11-4] Handwash sink used for purposes other than handwashing. Observed two scrapers in hand wash sink next to oven in main kitchen.
[36-34-5] Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed dusted vent over dry storage area.
[38-01-4] Light shield damaged/in disrepair. Observed flickering light over preparation area next to dry storage area.
[16-46-4] Old labels stuck to food containers after cleaning. Educated operator.
[29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Operator placed on mop sink faucet.
[41-10-4] Toxic substance/chemical improperly stored. De-limerick cleaner and oven cleaner placed on dry storage racks next to clear wrap. Operator moved items.
