  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler prep table with soiled gaskets.
  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed personal cell phone and bag stored next to rack full of seasonings and spices in back prep area. Operator removed.
  • [02D-01-5] Working containers of food removed from original container not identified by common name. Observed clear squeeze bottles containing sauces, oils, and dressings with no label near front counter.
  • [21-12-4] Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloth on top of cutting board in prep area not stored in sanitation solution. Operator removed.
  • [12A-28-4] Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee at the cook line touch soiled apron and then a clean set of tongs without changing gloves or washing hands. Coached employee on proper hand washing.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed black beans (63F - Cold Holding); corn (47F - Cold Holding) inside reach in cooler located across main prep table. As per operator, items have been there for about an hour. Operator placed inside walk in cooler for rapid cooling.
  • [41-10-4] Toxic substance/chemical improperly stored. Observed bottle of degreaser stored next to clean kitchen equipment in back storage area. Operator removed.
