Operator reheated fish to more than 165F

[12A-17-4] Employee rubbed hands together for less than 10-15 seconds while washing hands. Employee came in from exterior and washed hands for a total of less than 10 seconds. Discussed with person in charge. Person in charge discussed with employee.

Sliced white American cheese (49F - Cold Holding); hot dogs (46F - Cold Holding) in walk-in-cooler. Person in charge stated he was in and out of the walk-in-cooler a lot this morning. Operator opened inner walk in freezer door during this inspection to chill foods. Retemped at end of inspection and TCS foods 43F and lower.
[31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash sink in employee restroom. Operator replenished paper towels.
[53B-01-5] No proof of required state approved employee training provided for any employees. Person in charge unable to provide proof of employee food safety training. Only CFM certificates for management were available.
