[29-08-4] Plumbing system in disrepair. Observed hand wash sink on front service line covered with do not use sign. Operator stated they are waiting on a part to repair hand wash sink.
[16-46-4] Old labels stuck to food containers after cleaning. Observed multiple clean pans on storage shelves in warewashing area still have old label adhered to pan/ container.
[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed interior of chip machine on front service line containing chips soiled. Also, underside of mixer in prep area soiled.

[41-10-4] Toxic substance/chemical improperly stored. Observed container of hand soap stored next to oil on shelf below thermalizer in prep area. Operator removed container of hand soap.

Also, interior of hand wash sink on cook line soiled. Also, exterior of equipment on cook line and prep area (fryers, reach-in coolers, drawers, bump bars and cords for equipment, etc) soiled. Also, exterior of multiple seasoning bins in prep area and dry food storage area soiled. Also, exterior of paper towel dispenser on cook line near breading station soiled. Also, filters in hood above fryers and grill soiled.

Disposition: Administrative complaint recommended
[21-08-4] Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer Bucket (Quaternary 0ppm); on cook line, and front service line. Observed server had just made solution, tested at 0ppm. Observed bottle of sanitizer at triple sink empty. Discussed with operator about empty container of quaternary solution at triple sink. Operator changed out container and remade solution , tested at 200ppm.
[24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. White bowls and plates in back prep area shelves not inverted.
[22-08-4] Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of turbo oven and double oven in prep area have accumulation of debris. Interior of microwaves on cook line soiled.
[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on multiple reach-in coolers on cook line and front service line soiled. Also, exterior fronts and sides of multiple pieces of equipment on cook line, and front service line soiled. (Fryers, reach-in coolers, drawers, etc.) Also, bump bar on cook line next to breading station soiled/ accumulation of debris.
[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. meatloaf (42F-47F - Cold Holding); in reach-in cooler drawer across from grill area and raw salmon (43F-49F - Cold Holding) in reach-in cooler drawer below grill. Discussed with operator that same foods in back of drawer in temp, but those in front of drawer near opening were out of temperature, the drawer may not have been fully closed during rush. Operator placed bags of ice on both products to chill.

Two employees (Emily, Sarymar) food safety certification expired 2024. Operator stated they are working on completion.
[13-02-4] Employee with ineffective hair restraint while engaging in food preparation. Multiple employees pulling food at expo line, changing out soda boxes, carrying trays of plated food, and making beverages have ineffective hair restraint.
