[14-11-5] Equipment in poor repair. Observed door on large ice machine in disrepair. Also, gasket on reach-in cooler drawers torn. Also, interior doors on reach-in coolers across from oven and grill cracked/ in disrepair.
[29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed Vacuum breaker missing from non chemical side of splitter at mop sink.
[12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee beverage on prep table and next to microwave on cook line. Employee removed beverages in both places to properly store.
[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed multiple gaskets on reach-in coolers and reach in freezers on cook line and front area soiled. Also, gasket on walk-in-cooler soiled.
[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed ice deflector plate in large ice machine soiled. Also, underside of shake machine at prep table soiled.
[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sliced Swiss cheese, sliced American cheese (46F-47F - Cold Holding) in top of flip top reach-in cooler across from oven. Observed cheese stacked higher than chill line of pan. Employee removed half amount of cheese and placed in reach-in cooler to chill.
[41-10-4] Toxic substance/chemical improperly stored. Observed spray bottle containing toxic substance stored on prep table next to shake machine, and also a spray bottle of same substance stored on top of single use to go cups in cabinet below self dispensing soda machine. Operator removed both spray bottles of cleaners to properly store.
