- [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed Vacuum breaker missing from non chemical side of splitter at mop sink.
- [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee beverage on prep table and next to microwave on cook line. Employee removed beverages in both places to properly store.

Employee removed half amount of cheese and placed in reach-in cooler to chill.

Observed spray bottle containing toxic substance stored on prep table next to shake machine, and also a spray bottle of same substance stored on top of single use to go cups in cabinet below self dispensing soda machine. Operator removed both spray bottles of cleaners to properly store.
