Observed nozzles on juice machine soiled. Operator stated the machine no longer works and he plans to discard it.
[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets on flip top reach-in cooler and on cooler across from entry door soiled. Also, observed side exterior of reach-in cooler next to grill has an accumulation of grease build up.
[31A-11-4] Handwash sink used for purposes other than handwashing. Observed hand wash sink being used to store large bucket. Operator removed bucket from hand wash sink.
[41-17-4] Spray bottle containing toxic substance not labeled. Observed Unlabeled Spray bottle containing toxic substance stored on shelf below food pick up window. Operator labeled spray bottle of window cleaner.
[50-04-4] Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. Observed no license # affixed on exterior of MFDV.
