  • [24-08-4] Equipment and utensils not properly air-dried - wet nesting.
  • [32-05-4] Bathroom facility in disrepair. Sink is off the wall in the handicap stall in the ladies bathroom. The door is locked.
  • [36-73-4] Floor soiled/has accumulation of debris. Debris build up on the floor on the side of the beverage syrup rack in the dry storage area.
  • [16-21-4] Accumulation of debris on exterior of warewashing machine. Bottom of the dishwasher doors has a white build up.
  • [25-02-4] Unwrapped single-service utensils not presented so that only the handles are touched. Blue spoons on the front counter. The manager had an employee to present the spoons with the handles first.
  • [12A-09-4] Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Employee picked up an item off the floor with gloves on and then engaged in making a frozen custard. I informed the manager and he had the employee to remove the glove wash his hands and put on a new pair of gloves.
