  • Disposition: Warning Issued
  • [51-11-4] Carbon dioxide/helium tanks not adequately secured. Three co2 tanks not secure beside fence behind building.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Exterior of ice machine door soiled with mold-like substance.
  • [16-46-4] Old labels stuck to food containers after cleaning. Remnants of old labels stuck to sides of three pans on clean dish rack in back of kitchen.
  • [33-16-4] Open dumpster lid. Lid of dumpster left open.
  • [06-01-5] Time/temperature control for safety food thawed in an improper manner. Five pans of frozen fish and shrimp thawing at room temperature in rear prep room. All items are still frozen. The manager moved the items back into a cooler to continue to thaw.
  • [33-30-4] Unnecessary items stored in garbage enclosure. Discarded mattress, freezer, and other appliances stored beside dumpster.
  • [35A-02-7] Live, small flying insects found. 1. Four flies flying in kitchen at cookline. 2. One fly flying at front of kitchen.
  • [08A-02-6] Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Open box containing raw frozen beef stored on shelf above portions of fully-cooked frozen crab and portions of frozen fruit. Manager moved the raw beef to the bottom shelf during this inspection.
  • [41-10-4] Toxic substance/chemical improperly stored. 1. Spray bottle containing surface cleaner stored on prep table at cookline beside hamburger buns. Manager removed the chemical from the table during this inspection. 2. Spray bottle containing glass cleaner on shelf in storage room beside seasonings and above single-use items. Manager removed the glass cleaner from the area during this inspection.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. 1. Backflow prevention device missing at both ends of splitter added to spigot above mop sink. 2. Backflow prevention device missing at spigot on back of building. 3. Backflow prevention device missing at hose connection behind dumpster.
  • [01C-05-4] Clam/mussel/oyster tags not maintained in chronological order according to the last date they were served in the establishment. Two clam tags with hand-written dates of 4/1 and 4/3 stored above tags with date mark of 4/23, 4/19, and 4/22 on tag spike provided by manager.
