  • [14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Observed at dessert area reach in freezer.
  • [31A-11-4] Handwash sink used for purposes other than handwashing. Observed front line waiter station hand sink used as dump sink with evidence of coffee dumped inside. Operator cleaned.
  • [36-73-4] Floor soiled/has accumulation of debris. Observed throughout kitchen area.
  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee bags stored on prep shelf at dessert station. Operator removed.
  • [42-01-4] Wet mop not stored in a manner to allow the mop to dry. Observed stored in mop bucket at mop sink.
  • [51-11-4] Carbon dioxide/helium tanks not adequately secured. Observed not secured at coffee prep station of kitchen.
  • [08B-12-5] Stored food not covered. Observed at cook line reach in cooler, containers of steaks and asparagus stored uncovered. Observed at dessert station reach in cooler, containers of ice cream and gelatos stored uncovered.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed none at mop sink with splitter added.
  • [24-27-4] Clean equipment/dishware/utensils stored next to handwash/food preparation sink exposed to splash. Observed at front line waiter station container of sauce dishes stored next to hand wash sink. Operator removed.
  • [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employees energy drink can stored on cook line shelf. Operator removed.
  • [03D-02-5] Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed at cook line reach in cooler, mashed potatoes (50F - Cooling). As per label from 8/4/26. Operator removed and discarded.
  • [22-41-4] Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed at main dish machine, Dishwasher(Chlorine 10ppm). Operator began to replace empty container.
  • [10-17-4] In-use knife/knives stored in cracks between pieces of equipment. Observed stored between coolers at cook line. Operator removed.
  • [06-09-2] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. See stop sale. Observed at walk in cooler frozen packages of mahi placed to begin thawing. Operator began to remove fishes from packaging to properly thaw.
