Observed Raw chicken (56 F - Cold Holding); beef (56 F - Cold Holding); cheese (69 F - Cold Holding); pork (58 F - Cold Holding)inside the reach in cooler at the sandwich station. Also observed breaded chicken (56 F - Cold Holding); breaded pork (56 F - Cold Holding) at the kitchen area by the fryer. As per employee foods were placed yesterday night. Foods have been out of temperature for more than 4 hrs. Stop sale was issued.

Observed fried rice (122 F - Hot Holding); yellow rice (109 F - Hot Holding) inside the hot box. As per manager foods were stored less than 2 hours ago. Manager reheated the food to 165°F.

Observed reach in cooler at the sandwich station is not working properly. All foods are out of temperature. Also observed reach in cooler at the kitchen by the fryer is not working properly.
[36-12-4] Floors not constructed to be easily cleanable. Observed floor tiles missing grout in some areas of the establishment.
