  • [06-09-1] Commercially processed reduced oxygen packaged salmon bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package, observed thawing in walk in cooler, operator placed in walk in cooler less than one hour ago. Operator removed from packaging.
  • [08A-20-5] Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed container of raw chicken stored on shelf directly above bucket of fresh cut potato fries in water in single door reach in cooler in kitchen. Operator reorganized.
  • [22-20-5] Accumulation of black mold-like substance in the interior of the ice machine.
  • [21-10-4] Soiled dry wiping cloth in use. Observed dry wiping cloth soiled with food debris in use next to steam table.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Observed tray of raw shell eggs stored on shelf directly above canned drinks. Operator reorganized.
