- [14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up in the walk in freezer.
- [14-17-4] Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed rust on walk in cooler shelves. Operator stated they are in the process of replacing them.
- [51-13-4] No Heimlich maneuver/choking sign posted. Observed choking poster not displayed operator had one and put it on the office door.
- [14-11-5] Equipment in poor repair. Observed bottom of walk in freezer door damaged.
