  • [16-46-4] Old labels stuck to food containers after cleaning. Old label/label residue on clean bins on rack across from three compartment sink. Manager removed for cleaning.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Prep reach-in cooler cutting board on cookline soiled with mold-like substances.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Soda machine backsplash soiled with mold-like substances.
  • [12A-07-5] Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee touched arm/shirt sleeve, donned gloves, and continued food preparation without washing hands. Manager hand employee wash hands and change gloves.
