Operator provided proof of time products were placed, added time mark to white board next to office.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets soiled at cook line. Observed grease buildup on side of reach in freezer at cook line.
  • [36-22-4] Floor area(s) covered with standing water. Observed standing water on floor at salad prep reach in cooler.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter at mop sink missing vacuum breaker on one side.
  • [06-09-1] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed packages of raw salmon and raw mahi thawing inside reduced oxygen packaging inside meat walk in cooler. Advised operator to cut open bags before product thaws.
  • [14-17-4] Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed walk in cooler shelves rusted.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed bun rack soiled with old food debris next to cook line.
  • [14-33-4] Reach-in cooler/freezer shelves with rust that has pitted the surface. Observed rusted shelves inside reach in freezer next to fryers and inside reach in cooler in back prep area.
  • [16-37-1] No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed no chemical test kit for chlorine.
