  • [25-05-4] Single-service articles improperly stored. Observed open box of dome lids stored on floor of kitchen. Manager removed and stored properly.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter added to mop sink without back flow device on non chemical side of splitter. Manager removed splitter from sink.
  • [31A-09-4] Handwash sink not accessible for employee use at all times. Observed employee hand washing sink stored with personal items. Manager removed and stored properly.
