- [10-07-4] In-use utensil stored in standing water less than 135 degrees Fahrenheit. Ladle in water at 95f, educated employee, moved to griddle to increase temperature to 135f.
- [14-05-4] Cardboard used to line food-contact shelves. Cardboard used to line shelf in walk in cooler under uncut deli meats. Removed.

Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Tested twice at 0 ppm, bucket 75% full, line or prime not working. Triple sink set up. Primed machine retested at 50ppm.

- [55-01-5] Establishment charging operational charges without properly disclosing the information on the menu/web/app and the bill/receipt. Operator charges a credit card fee, to be in compliance with new law it must be posted on the menu and website. Currently only on cash register.
