Raw fish over cooked shrimp at reach-in cooler in sushi bar area. Operator rearranged.

- [13-03-4] Employee with no hair restraint while engaging in food preparation. Several employees engaged in food preparation in kitchen not wearing a hair restraint. Operator had them put on hats.
