[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Checked spinach 45F stored inside reach in cooler less than 4 hours recommend to not stacked bags on top each other quick chill in the freezer.
[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed white cutting board soiled has red discoloration on the surface located salad prep cooler,
[35A-02-8] Live, small flying insects found. Observed one flying insects in the dish machine , one ice machine also one in expo line (server area) Notify Supervisor on duty next unannounced inspection.
[06-09-2] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed salmon unthawed in ROP package only slit the package stored inside walk in cooler, Spoke to Manager concerning this issue employee had slit package this morning.

[13-04-4] Employee with no beard guard/restraint while engaging in food preparation. Observed employees cooking on the line without wearing a beard nets.
[14-11-5] Equipment in poor repair. Observed fire suppression system has broken hood filters.(four) Manager provided proof of order new hood filters.
[36-73-4] Floor soiled/has accumulation of debris. Observed old food debris throughout the kitchen floor tiles.
[33-20-4] Grease on the ground and/or pad around grease receptacle.
[14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Observed large build up of ice inside reach in freezer located on cook line.
[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed old food debris underneath the flat grill on the cook line.
[14-33-4] Reach-in cooler shelves with rust that has pitted the surface. Observed rusty storage shelves inside reach in cooler located back prep kitchen also on the cook line.
[36-27-5] Wall soiled with accumulated grease, food debris, and/or dust. Observed walls near the ice machine top panel has mold like growth.
[03D-02-5] Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Checked temperature of cooked bacon 48°f stored overnight inside walk in cooler prepared labeled @ 6:01 pm on Monday 3/23 did not cool within 6 hours. Issued a stop sale notice on the cooked bacon. Ask for restaurant procedures for cooking bacon found out must be cooked to 145°f, recommend Manager to follow correct procedures.
[01B-02-5] Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cooked bacon stored inside reach in cooler 48°f Labeled 3/23. Issued stop sale notice.
[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed mixer has old food debris left on the top panel. Observed green cutting boards soiled used in the preparation area.
[14-16-4] Multiuse food-contact surface not smooth, free of breaks/cracks/chips/pits/crevices. Observed cracked plastic container stacked on the top of the ice machine, Manager removed.
