  • [36-37-5] Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Observed ceiling kitchen area above food warmer and reach-in cooler and reach in freezer not smooth, non absorbent and easily cleanable.
  • [22-08-4] Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed interior of food warmer with clear doors in kitchen soiled.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed exterior of fryers and oven soiled. Also, filter above prep table soiled.
  • [29-20-5] Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Observed mop sink very slow draining. Operator stated a plumber has been called.
  • [35A-02-8] Live, small flying insects found. Observed one flying insect at triple sink.
  • [08A-17-6] Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Observed raw ox tail stored above raw fish in reach in freezer in kitchen-not all products commercially packaged. Operator rearranged to properly store fish.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed vacuum breaker missing from non chemical side of splitter at mop sink. Operator installed vacuum breaker during this inspection.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed portable white cutting board next to triple sink soiled/ stained. Also, blade of can opener soiled.
