  • [41-17-4] Spray bottle containing toxic substance not labeled. Observed spray bottle with bleach not labeled.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
  • [22-41-4] Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dishwasher machine at bar area not working properly. Dishwasher (Chlorine 0ppm). At both bar area and back kitchen. Employee set up Triple Sink (Chlorine 100ppm).

  • [36-36-4] Ceiling tile missing. Observed above entrance hallway kitchen,prep area.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed cutting board across cook line soiled mold-like substance.
  • [01C-03-4] Clam/mussel/oyster tags not marked with last date served.
  • [53B-01-5] No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed no certificate for Shaun, Yves, Antwon, Vincent, Alexis, Johanna.
