  • [14-01-5] Bowl or other container with no handle used to dispense food. Bowl in rice and sauce. Manager removed all.
  • [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drinks on shelf over prep table on cook line. Moved during this inspection.
  • [08B-48-4] Employee preparing food in customer section of dining area. Employee putting sauce in cups at front counter. Manager instructed employee to move to kitchen.
  • [12A-12-4] Employee switched from working with raw food to ready-to-eat food without washing hands. Male employee used squeegee to remove juice from raw chicken of table then flipped rice into pan without washing hands. Spoke with manager.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. In walk in cooler: pan of raw shrimp on shelf over sauce.
  • [31A-09-4] Handwash sink not accessible for employee use at all times. Sink in ware wash area has trash can in front of it. Moved during this inspection.
  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. At kitchen entrance. Filled during this inspection.
  • [03F-10-5] No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Using Time as a Public Health Control (TPHC) for sushi rice and shrimp tempura.
