  • [10-06-5] In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed handle of scoop touching the cheese at the prep table cooler unit.
  • [25-05-4] Single-service articles improperly stored. Observed boxes of SS items such as cup covers stored on the floor in the front counter. Employee removed.
  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. Observed for handwash sink in the back. Employee provided.
  • [33-15-4] Garbage can located outside has no lid or lid open/broken.
  • [08B-47-4] Food not stored at least 6 inches off of the floor. Observed 4 boxes of harina and three 2.5 gallons not stored 6in off the floor.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breaker for hose bib with hose next to the handwash sink in the kitchen.
  • [14-11-5] Equipment in poor repair. Observed left door of RIF located near bathroom, comes off the door hinge when opening
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled door gaskets and door frame of reach in freezer across of stove and drawer gaskets of RIC at cook line.
  • [03B-15-5] Hot time/temperature control for safety food received at less than 135 degrees Fahrenheit. Observed bean empanada (127F - Hot Holding, per operator cooked around noon). Operator reheated empanada, 2nd temp. 148°F.
  • [14-31-5] Nonfood-grade bags used in direct contact with food. Observed cooked shredded beef stored inside a non food grade bag (plastic grocery bag) inside the walk in cooler.
  • [12A-07-5] Employee handled boxes from the floor and failed to remove dirty gloves and wash hands before putting on new gloves to initiate a task working with food. Employee was told, she removed gloves, wash hands and replace to new gloves.
  • [08A-08-5] Raw animal food stored in same container as ready-to-eat food. Observed raw bacon stored inside the same food pan with ready to eat tomato, onion, cheese and lettuce. All the food items were in contact with each other. Employee discarded.
  • [12A-29-4] Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee clean up a broken raw shelled eggs and started wiping down prep counter and about to engage in food handling. Stopped employee prior to and asked her to wash her hands.
  • [36-08-4] Floor and wall junctures not coved in food preparation/storage/warewashing areas or bathroom. Observed missing coving from wall to handwash sink located in front area.
