  • [14-11-5] Equipment in poor repair. Multiple gaskets on reach in coolers in establishment torn. Also, flip top reach in cooler at front counter has duct tape on lid/handle.
  • [33-16-4] Open dumpster lid. Dumpster lid open. Operator will send employee out to close it.
  • [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine soiled with white/black mold-like substance. Operator began to clean.
  • [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee Gatorade bottle on top shelf of metal rack behind registers above single service items. Operator moved employee drink. Also, employee mug on back prep table next to single service items and tomatoes. Operator had employee move cup.
  • [24-08-4] Equipment and utensils not properly air-dried - wet nesting. On clean dish rack, multiple pans stacked before properly air drying.

In front counter reach in cooler, cooked onions (70F - Cooling @ 7 hours). Operator stated onions were done at 8:10 am and put in 1 gallon plastic container with lid and put in bottom of cooler. Temperature taken at 3 pm. See stop sale.
  • [36-73-4] Floor, wall and/or ceiling soiled/has accumulation of debris. On ceiling vents at either end of front counter soiled with dust build up. Also, lower wall under 3 compartment sink soiled with residue.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In top of flip top reach in cooler on front counter, shell less hard boiled egg (57F - Cold Holding). Eggs are in pan with water with lid open right in front of the exit side of toaster oven. Eggs were stocked no more than 1 hour ago. Operator put eggs in bottom of reach in cooler to bring temperature down.
  • [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Ice bucket on top of ice machine not inverted. Operator flipped bucket over.
  • [22-08-4] Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of microwave at front counter soiled with food debris.
