- [03B-01-6] Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed shredded beef (78F - Hot Holding); chickpeas soup (87F - Hot Holding), in steam table. As per employee food was placed in steam table 2 hours before the inspection. During the inspection, the employee reheated the items to proper temperatures: shredded beef to 169°F and chickpea soup to 171°F.
- [14-17-4] Walk-in freezer shelves with rust that has pitted the surface.
- [23-24-4] Buildup of food debris/soil residue on equipment door handles. Observed multiple kitchen equipment doors handles soiled.
- [22-16-4] Reach-in cooler interior/shelves have accumulation of soil residues. Observed 2 doors reach in cooler interior soiled.
- [42-01-4] Wet mop not stored in a manner to allow the mop to dry.
- [06-09-1] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed raw fish thawing in standing water in the handwashing sink located in the preparation kitchen. During the inspection employee removed fish and placed it in reach in cooler.
- [27-16-4] Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee hand sink. Observed that the handwashing sink located in the preparation kitchen did not have running hot water available. The issue was corrected during the inspection, and running water was restored by the employee.
- [24-05-4] Clean glasses, bowls, and plates not stored inverted or in a protected manner.
- [29-11-4] Water leaking from pipe and/or faucet/handle. Observed that the pipe underneath the handwashing sink in the preparation kitchen is leaking water.
- [08A-24-5] Raw animal foods not properly separated from each other during preparation based upon minimum required cooking temperature. Observed raw chicken and raw pork thawing together in the sink in the preparation kitchen. During the inspection employee removed items.
- [36-27-5] Wall soiled with accumulated grease, food debris, and/or dust, throughout the kitchen areas.
- [12A-03-4] Employee washed hands in a sink other than an approved handwash sink. Observed employee washing hands in 3 compartment sink.
- [29-08-4] Plumbing system in disrepair. Observed that the handwashing sink located in the preparation kitchen did not have running water available at the time of inspection. The issue was corrected during the inspection, and running water was restored by the employee.
