  • [06-01-5] Time/temperature control for safety food thawed in an improper manner. Two ziplock bags of raw chicken thawing in triple sink in pot of standing water. Operator turned cold water on to run over product.
  • [14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Ice building under fan unit in stand up reach in freezer.
  • [36-73-4] Floor, wall and/or ceiling soiled/has accumulation of debris. Floor and wall under/behind dish area soiled with food debris. Also, wall behind ice machine dusty. Also, ceiling above back door in hallway soiled with dust.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple gaskets throughout establishment soiled with food debris/torn. Also, Portable fan on cook line has dust build up.
  • [35B-01-4] Exterior door has a gap at the threshold that opens to the outside. Exterior back door in hallway of bathrooms has gap at lower left corner with light coming through.
