  • [14-09-4] Cutting board on front counter has cut marks and is no longer cleanable.
  • [21-12-4] Operator continuing to store wet wiping cloths outside sanitize solution. Discussed proper storage of wiping cloths, in sanitize solution.
  • [12A-13-4] Operator used gloved hands to handle dirty dishes, fill out paperwork, and then to clean inside of ice machine. -Operator used gloved hands to sweep up dead roach and handled broom and dustpan, and proceeded to engage in preparation of boba.
  • [11-07-5] Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items.
  • [53A-10-4] Certified food manager unable to answer basic Food Code questions pertaining to safe operation of establishment. Operator claimed that she did not know that she needed to use soap when washing hands, was reluctant to wash hands, continuously refused to wash hands. Did not know proper handwashing methods. Discussed proper handwashing at length with operator.
  • [31A-11-4] Reusable gloves used for dish washing, cleaning, food prep, being stored on hand wash sink next to 3 compartment sink
