- [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink located next to fire grill. Operator provided paper towels.
- [36-17-5] Floor tiles missing and/or in disrepair in the main kitchen area near the dish machine.
- [52-04-5] Establishment advertised a specific fish on the menu but served another type of fish. On the menu, dish "traditional ceviche" specifies fresh corvina. Operator could not provide any invoices of corvina fish. Operator stated they are serving swai, operator provided invoices with swai listed. Operator stated they will be reprinting menus.

On the menu under Guacho starters observed "traditional ceviche" dish, operator stated raw corvina is marinated in lime juice without any heat applied. Operator began to mark menu items to indicate their consumer advisory.
