  • [22-31-4] Non-pitting surface rust on food-contact equipment. Observed rust on bottom shelf of prep table.
  • [36-32-5] Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tile above prep table missing.
  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee phone stored on cutting board on cook line.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed hose bibb utilizing splitter with no vacuum breaker attached.
  • [45-05-4] Portable fire extinguisher missing from its designated location. For reporting purposes only. Observed fire extinguisher in server area stored on floor.
  • [16-55-4] Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed dish machine in kitchen area measuring (Chlorine 0ppm). Chef set up three compartment sink with a new ready of (Quaternary 200ppm). Chef call dish machine technician.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw beef stored over ready to eat Pico De Gallo.
  • [25-05-4] Single-service articles improperly stored. Observed single service pizza boxes stored on floor of dry storage area.
