  • [36-51-4] Building components, attachments or fixtures in poor repair. 1. Cracked wall tile beside ice machine. 2. Holes in wall on outside of walk-in cooler.
  • [14-11-5] Equipment in poor repair. Cracked plastic around door of ice machine.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Area around nozzles of fountain machine in lobby soiled with mold-like substance. An employee began cleaning the area during this inspection.
  • [42-03-5] Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Screwdriver stored on top of reach-in cooler beside grill. Manager removed the screwdriver from the area during this inspection.
  • [35A-02-7] Live, small flying insects found. One fly flying in kitchen at cookline. Two flies flying in kitchen near drive through window. One fly flying in kitchen near ice machine. One fly flying in kitchen in dry storage area.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Backflow prevention device missing after splitter added to mop sink. Manager removed the splitter from the mop sink during this inspection.
  • [53B-16-4] Employee has not received adequate training related to their assigned duties as evidenced by the inability to answer basic food safety questions. Employee at grill unable to give minimum cooking temperature requirements for hamburger meat. Inspector educated employee during this inspection.
