- [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed : 1 cell phone stored at the back prep table near the office. The phone was moved to the office.
- [35B-01-4] Exterior door has a gap at the threshold that opens to the outside. Observed : the walk-in freezer has an exterior exit/entrance, this has a gap along the whole bottom edge of the door.

- [31A-09-4] Handwash sink not accessible for employee use at all times. Observed : the drive thru hand wash sink was blocked with a rolling cart. The cart was moved.
- [53B-10-4] Records/documents for required employee training do not contain all of the required information. Observed : Employee Training records are not signed off by a manager. The manager started signing the forms from the McDonalds training.
