  • [31A-09-4] Handwash sink not accessible for employee use at all times. At prep area, handwash sink across from ice machine has cart in front of sink. At wait station, handwash sink has coffee pot in front of sink. Operator moved items from blocking sinks.
  • [35A-02-7] Live, small flying insects found At dish area, observed 1 flying insect.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. At mop sink, backflow preventer located on faucet above splitter. Baclflow preventer should be located at end of splitter on non chemical dispenser side before attached hose.
  • [08B-17-4] Unwashed fruits/vegetables stored with ready-to-eat food. In walk-in cooler, unwashed carrots and tomatoes stored over ready to eat spring mix and shredded vegetables. Operator moved items to proper storage.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. At prep area, bulk containers have debris build up on exterior. At prep area, gaskets on cvap oven soiled with food residue. At prep area, exterior of ice machine door has build up of residue.
  • [12B-13-4] Opened employee beverage container in a cold holding unit with food to be served to customers. In walk-in cooler, open employee bottled drink n shelf with customer foods. Operator moved to employee food storage shelf.
  • [53B-14-5] Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Employee training certificates for L. Oliver and B. Dyer expired.
