  • [12A-09-4] Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Employee cleaned shoe with paper towel and proceeded to handle clean to go container without washing hands and changing gloves. Manager had employee wash hands, change gloves and use new to go container.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. White cutting board on kitchen prep table. Shelves in reach-in cooler behind front counter.
  • [31A-11-4] Handwash sink used for purposes other than handwashing. Measuring cup stored in kitchen hand washing sink. Employee removed cup during inspection.
