- [41-10-4] Toxic substance/chemical improperly stored. Observed bathroom above wooden serving boards. Operator removed cleaner.

- [08B-38-4] Food stored on floor. Observed cases of fries on floor in walk in freezer. Operator removed items off of floor.
- [31A-09-4] Handwash sink not accessible for employee use at all times. Observed hand sink in ware wash area blocked. Operator moved shelf to access hand sink.
- [02D-01-5] Working containers of food removed from original container not identified by common name. Observed in dry storage; flour no label.
- [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cooler Across cook line with ambient temp of 54F. The following TCS foods stored inside for less than 2 hours: pico de gallo (60F - Cold Holding); cut tomato (57F - Cold Holding). Operator grabbed ice to place on items.

Observed employee rub face and continue work in kitchen without handwashing. Inspector spoke to operator about proper handwashing techniques, and provided handout.
