- [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed open employee drink on prep table. Employee moved it immediately.
- [13-03-4] Employee with no hair restraint while engaging in food preparation. Observed employees engaged in food preparation without hair restraint.
- [08B-38-4] Food stored on floor. Observed container with cooked potatoes on the floor. Employee transferred the potatoes immediately
- [31B-04-4] No handwashing sign provided at a hand sink used by food employees. Missing handwashing sign at the hand sink located at the entrance of the ware washing area. Employee posted it.
- [03F-02-5] Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking . Observed cut watermelon and cooked potatoes using time control ,but items were not marked. Establishment has the procedure. As per employee food has been there for 1 hr. Manager labeled the food.
- [41-10-4] Toxic substance/chemical improperly stored. Observed sanitizer bottle by tortillas and sauces in a rack. Manager moved it .
- [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed accumulation of debris on the can opener. Employee washed it immediately.
- [31B-03-4] No soap provided at handwash sink. Missing soap a the hand sink located at the kitchen area. Person in charge replaced the empty bottle immediately.
