[03D-15-4] Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed cooked noodles cooling at room temperature on speed rack by rear exit. Cooked noodles were at temperatures of 81F and cooling for 1.5 hours. Cooked noodles were placed in walk-in cooler.
[14-20-4] Ripped/worn tin foil used as shelf cover. Observed torn tin foil long in food contact shelf next to reach in cooler on cook line.

[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In first reach in cooler near front counter, tso chicken (47F - Cold Holding); sweet and sour chicken (47F - Cold Holding). Operator stated food was out while cooking orders then placed back into reach in cooler.
[10-06-5] In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. In flip top reach in cooler on cook line, handles of tongs in bowl of noodles touching product. Operator removed handle from product and lifted above edge of bowl.
[14-01-5] Bowl or other container with no handle used to dispense food. On bottom shelf of prep table, container of sugar and container of salt have to go bowl observed in product being used as scoop. Operator removed to go bowls.
[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Exterior of fryers have build up of food/grease debris. Also, multiple gaskets on reach in coolers and reach in freezer soiled/torn. Also, hood filters above cook line soiled with grease build up. Operator stated hood company is coming tomorrow to clean.
[25-02-4] Unwrapped single-service utensils not presented so that only the handles are touched. On table in lobby, single service forks and spoons stored in bottom drawer of plastic container not presented with handles first. Operator grabbed to large to go container and moved all forks and spoons so handles were presented first.
[42-01-4] Wet mop not stored in a manner to allow the mop to dry. Mop stored in dirty water in mop bucket in mop sink. Operator rang mop out and hung up and discarded of dirty mop water.
[36-73-4] Floor soiled/has accumulation of debris. Floors under fryers soiled with black build up and food debris. Also, floor under cook line/wok station soiled with debris. Also, floor under first reach in cooler near front counter soiled with food debris.
[12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Operators open beverage container on clean prep table. Operator removed his drink from table.
[35A-06-4] Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Dead roach and dead lizard observed in sticky trap on floor under reach in freezer. Operator discarded sticky traps.

In walk-in cooler, small metal bowl with two raw chicken breast stored on top of shelf above bowl of raw shrimp. Operator moved raw chicken breast with other raw chicken products.
[02D-01-5] Working containers of food removed from original container not identified by common name. On bottom shelf of prep table, three large containers of sugar, salt and msg removed from original packaging and not labeled. Operator labeled wash container.
[10-20-4] In-use tongs stored on equipment door handle between uses. In use tong hanging on magnetic hook observed at knee height on steam table. Operator moved magnetic hook to wall above steam table and stored tongs properly.
[08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. In walk-in cooler, operators personal raw shelled eggs stored on shelf above container of ready to eat sauce. Operator moved raw eggs next to restaurant eggs till he takes home today.
