  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Missing vacuum breaker on hose bib where hose is attached.
  • [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed black like substance inside ice machine in back area.
  • [02C-04-5] Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Observed meat sauce with no 7 day label. Per operator it was made on Sunday.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed accumulation of soil on shelves housing clean food pans and food equipment above 3CS. Observed soiled with slimy back substance on door gasket of RIF under meat slicer.
  • [01B-02-5] Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cooked spaghetti (55F - Cold Holding). Per cooked yesterday around 6pm. 20 portions.
  • [53B-15-4] Employee has not received adequate training related to their assigned duties as evidenced by lack of general knowledge in performing their assigned duties. Observed soap with chlorine inside sanitize basin at 3CS. Confirmed with operator. Operator was coached and set up properly.
  • [03D-02-5] Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed cooked spaghetti (55F - Cooling). Per cooked yesterday around 6pm. Stop sale was issued for 20 portions. Operator discarded food items.
  • [36-34-5] Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed accumulation of rust on ceiling vents above prep, pizza kitchen area and where single service items. Also observed water stains on riling tiles blvd warewashing area.
  • [16-13-5] Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed soap with chlorine inside sanitize basin at 3CS. Confirmed with operator. Operator was coached and set up properly.
